Job Responsibilities
- Provide consultations with PCPs to enhance understanding of the patient, provide decision support for treatment planning, and assist in implementing and monitoring biopsychosocial treatment plans.
- Identifies and evaluates health risk behavior(s) and provides brief, targeted interventions to support optimum mental, behavioral, emotional, and physical health using the preferred care model at Ponca Health clinic sites.
- Works cooperatively with clinic staff to provide patients with referrals for appropriate specialty.
- Make referrals for community mental health resources as appropriate.
- Maintain a therapy caseload utilizing Evidence-Based Treatments.
- Provide outpatient counseling to individuals and families needing short or intermediate-term mental health interventions.
- Provide outpatient group therapy as needed.
- Conduct mental health, substance use, and co-occurring assessments and provide recommendations.
- Complete all charting as needed and required by contracted funding sources, including assessments, progress notes, and billing.
- Participate in weekly case huddles.
- Participate in weekly supervision and case consultation with the Behavioral Health Team.
- Other duties as assigned, including, but not limited to, Assisting in provider training and education in the integrated behavioral health-primary care model, assisting with grant data collection, attending rant recipient award meetings, conducting Community Outreach, and participating in Behavioral Health quality improvement activities.
Relationships and Authority:
This position has no supervisory authority.
Working Environment and Physical Demands:
This position is in the professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, scanners, and photocopiers.
Requirements
Minimum:
- Master’s degree in social work or counseling.
- Provisionally Licensed Mental Health Provider
- Excellent people skills and ability to work collaboratively with primary care staff, patients, as well as manage behavioral health clinicians.
- Computer proficiency in Microsoft Word, Excel, and PowerPoint programs.
- Excellent written and oral communication skills.
- Valid driver’s license and be insurable.
- Successfully pass drug and alcohol testing, motor vehicle, education, character, and criminal background checks.
Preferred:
- 2+ years’ experience in providing individual, family, and group counseling.
- Fully licensed or Independently Licensed Mental Health Provider
- Licensed or Provisionally Licensed Drug and Alcohol Counselor
- One year of experience working with Native Americans or Native American organizations.
- Knowledge and understanding of Public Law 93-638″ Indian Self-Determination Act.
Tier 1: (Grade 10)-Provisional Licensure
Masters in Counseling or Social Work
Active and good standing Provisional Licensed Mental Health Provider
Tier 2: (Grade 12)
Masters in Counseling or Social Work
Active and good standing Licensed Mental Health Provider
Tier 3: (Grade 13)
Masters in Counseling in Social Work
Active and good standing Licensed Independent Mental Health Provider
Active and good standing Provisional or Licensed Alcohol and Drug Counselor
Three or more years experience as a Licensed Mental Health and Substance Abuse Counselor
